Clinical Skills
Able to perform primary skills of a haemodialysis nurse in relation to patient care.
- Participate as a member of the team in carrying out haemodialysis procedures.
- Able to perform needling of A-V fistulas, A-V graft and CVC care for initiating haemodialysis process as per Diaverum standards.
- Responsible that all dialysis equipment is safe, complete and fully operational before, during and after each use.
- Responsible to monitor and record patient’s vital signs during treatment and report any abnormality to Medical Director.
- Assist other staffs and doctors during treatment and examination of patients on treatment.
- Perform other duties as assigned by Cluster Head Nurse.
Management of staff
Provides assistance to staff in accordance with the goals of the organization.
- Lead a team of Nurses.
- Makes staff schedule changes to adjust for patient acuity levels and adequate clinic coverage.
- Maintains a flexible schedule to be available to staff members.
- Appoint suitable staff to all necessary clinic assignments.
- Ensures implementation and compliance with Clinical Policies and Procedures.
- Provides a climate in which staff can readily participate in change or innovations.
- Promotes open and participative communication among staff.
- Responsible for the clinic staff meetings.
- Counsels and advises employees
- Plan and support the integration of new staff.
- Responsible for new Employee Orientation and probation period Evaluation to an assigned Mentor.
- Conducts evaluation annually, assessing competency in clinical skills, adherence to protocols, and patient care standards.
- Evaluates together with clinic mentors all new staff nurses 1 month before probation period ends.
- Liaison with HR and accounting regarding: New staff employment, Payroll and overtime, Appraisal, and Disciplinary and grievance actions.
Management of the Clinic
Controls and directs the clinic to encourage quality patient care and staff satisfaction.
- Manage patient planning: Scheduling, Holiday dialysis coordination, Patient Flow and Laboratory tests.
- Prepares monthly reports according to Diaverum present standards concerning goals, achievements and problems of the clinic. Communicate with Country Nursing Manager on relevant matters.
- Responsible for the clinic Infection Prevention and Control programs, identify and assigns Infection Control staff nurse in-charge.
- Coordinate with Country Nursing Manager to ensure that audit action plans are completed.
- Work as part of the Operations Management Team.
- Collaborate with Operations on safety, cleanliness, and security standards for the clinic.
- Consumption control and alignment with budget.
- Regularly evaluates and monitors the use and replacement of equipment and supplies.
- Coordinate supplies ordering and reviews stock control with stock in-charge.
- Evaluates new products and leads training of new products.
- Uses supplies and resources in a cost-effective manner.
- Responsible for ensuring and monitoring water quality standard according to Country Regulations and Diaverum Water Policies.
- Maintenance of water plant, monitors and all other medical- as well as other equipment.
- Support clinic and country management.
- Keep up to date with country regulations.
- Support Corporate programs.
Management of Care
Coordinates and sets standards for the delivery of quality care.
- Assigns patient care according to patient needs and staff capabilities and qualifications.
- Ensures the utilization of quality assurance standards and protocols.
- Assists with the development of clinic specific standards, and provides input.
- Supervises and evaluates the quality of care through patient rounds and specific quality assurance projects.
- Serve as a member of the Quality Management Committee and Actively participate in the Quality Management Review meeting and follow-up.
- Reviews, approves and analyze incident reports.
- Involvement in clinic audits, other quality certification processes and support in accreditation of clinics.
- Acts as a point of liaison with medical and non-medical staff in coordinating multi-disciplinary activities.
- Communicate and liaise with hospitals and other healthcare facilities.
- Sets standards for and evaluates nursing documentation.
- Participates in patient and family education.
Management and Communication
Interacts effectively with patients, family and staff to maintain standards of professional care to patients and families.
- Promotes positive working relationships within the team.
- Demonstrates sensitivity to cultural and religious beliefs through a caring, respectful attitude, presentation and delivery.
- Demonstrates effective written and verbal communication skills using recognized medical terminology.
- Facilitate and support the integration of new patients.
- Ensure patients receive the appropriate information and education.
Educational Responsibilities
Promotes professional growth and development of nursing staff
- Provides input to assist in identifying the learning needs of staff.
- Participates actively in the process to coordinate on-going professional development of staff.
- Responsible for the planning of continuous staff clinic education, the yearly in-service schedule.
- Responsible for maintaining all documentation necessary for the nurses and education and ensure necessary documents are appropriately signed and kept in the clinic level.
- Supervises the d.Academy Programme.
- Responsible for informing the Country Nursing Director and Human Resource (HR) Manager when employees complete the education programs and forwarding all required education documentation to the Country HR Manager.
REQUIREMENTS & QUALIFICATIONS
- Minimum 2-4 years of related experience
- Diploma/ degree in Nursing or equivalent
- Registered with Singapore Nursing board
- Experience in renal care will be an added advantage.
- Excellent English communication skills with the ability to handle inevitable objections, inspire trust and generate project enthusiasm.
- Must be able to commit to rotating shifts & weekends.
