JOB OVERVIEW AND KEY RESPONSIBILITIES

The following duties and responsibilities reflect the expectations of the position of the Head of Pharmaceutical Care. This is complemented and further explained in the role objectives set by the organization.

 

LEADERSHIP

  • Aim to cultivate a pharmaceutical care department that is patient-centric, evidence-based and progressively integrated within Diaverum care delivery system.

  • Create an impactful pharmaceutical medical care delivery system that exists within a multidisciplinary team (MDT) framework to ensure effective medical pharmaceutical care, quality of life and lifestyle readjustments for patients and their families including drug-drug interaction, drug-food interaction, Improve medications safety and improve adherence to medication, as well as enhancing patient experience.

  • To initiate and take the lead in managing defined pharmaceutical care projects and other strategic projects as appropriate.

  • Direct and coordinate activities of clinical pharmacists in coordination with Clinic Medical Directors and Area Managers.

  • Utilize department resources through appropriate planning, evaluation and data analysis.

  • Take an active role in following up and implement initiatives to improve patient medical outcomes (including services provided outside of clinics) by coordination with other functional heads, other government institutions, and holiday dialysis locally and internationally.

  • To demonstrate the ability to analyze complaints about medications effectiveness level and to provide guidance to staff as necessary.

  • To demonstrate the ability to investigate and analyze complaints and ethical issues and to provide guidance to staff as necessary.

 

PHARMACEUTICAL /MEDICAL/CLINICAL PERFORMANCE

  • Ensure the implementation of a continuous, accurate and effective pharmaceutical care plan/reconciliation for all patients through the clinical pharmacists. The pharmaceutical care plan includes screening, assessment, diagnosis, intervention/counseling, documentation, monitoring, and evaluation with focus on medications use and medical aspects.

  • Support research initiatives in clinics while ensuring their alignment with Diaverum pharmaceutical care department strategy and vision.

  • Work closely with the Corporate Medical Office in delivering highest medical quality and best IPPS performance aiming at meeting Diaverum corporate targets with focus on compliance to treatment modality.

  • Work closely under supervision of Country Medical Director in improving medical outcome at clinics by implementing latest pharmaceutical practice.

  • Ensure effective pharmaceutical care, health promotion, and lifestyle management supporting hemodialysis patients with different demographic backgrounds (gender, age, educational level).

  • Manage the set medical parameters to accurately measure, monitor and evaluate all aspects of pharmaceutical care in accordance to national and international standards and guidelines in renal care and chronic disease management.

  • Utilize medical knowledge and counseling skills to make a positive difference in patients’ quality of life.

 

POLICIES AND PROCEDURES

  • Ensure country procedures adhere to all Diaverum policies and procedures, and to country, regional and local regulatory requirements.

  • Be responsible for the writing, revision, communication, and implementation of country Pharmaceutical care/medications procedures with the aim of standardizing practices within all clinics.

  • Understand and comply with policies and procedures related to scope of care, patient confidentiality/rights, infection control, facility management and safety, as well adhering to time/attendance and dress code requirements.

  • Monitor compliance according to all pharmaceutical/medical care policies and procedures and country regulations.

  • Maintain an up-to-date reference file with scientific literature, country regulations, and other relevant information relating to country procedures.

  • Monitor compliance with all clinical policies and procedures and country regulations within clinics.

  • Communicate any desired changes of corporate clinical policies and procedures to the Diaverum Policy Task Force.

  • Work with the country operations team as appropriate to support implementation of operational policies and procedures at clinic level. Maintains international standards such as JCIA, ISO, etc.

  • Monitor current policies and procedures and future updates in Diaverum including quality standards such as Joint Commission International (JCI) and/or Saudi Central Board for Accreditation of Healthcare Institutions (CBAHI), medical KPIs as well as all local regulatory requirements governing pharmaceutical care.

 

PHARMACEUTICAL SUPPLY

  • Ensure all pharmaceutical supplies are within the local regulatory authority system e.g.; all medication approved by SFDA, generic products to be sure as effective as the original brands.

  • Take a lead in empowering pharmacists to manage pharmaceutical supplies to ensure medications safety, quality, availability.

  • Review all stock data that is related to slow-moving stock of drug items, overstock of drug items, nearly expired stock of drug items, and zero stock of drug items and give recommendations.

  • Coordinate with clinical pharmacists / Logistics Services team to prevent the waste of pharmaceutical supplies and improve pharmaceutical care services.

  • Coordinate with clinical pharmacists / Logistics Services team to ensure the availability of medications in the clinic at all times.

  • Be the point of contact for medical-operations amongst other departments with all pharmaceutical supplies matters concerning the department.

  • Make sure to report any item recall to ensure patient safety all around our clinic.

  • Permanent member of product board committee.

 

QUALITY MANAGEMENT

  • To present information (project proposal, implementation and evaluation strategy, project outcomes) to manage key stakeholders and department.

  • Influencing, motivating, supporting, and enabling the team to work towards changes.

  • Responsible for auditing of pharmaceutical care practice in all clinics and develop quality improvement plan.

  • Support, facilitate, and participate in quality assurance activities within the country.

  • Participate in Quality Management Review process when needed.

  • Work closely with the Clinic Medical Director, Clinical Pharmacist and Operations Director/Manager to assist, support and supervise the clinics in achieving overall quality goals and parameters.

  • Participate in country quality meetings as required.

  • Annual report of all pharmacy incidences, activities, and future plans.

 

CLINICAL PHARMACIST LEARNING AND DEVELOPMENT

  • To present information (project proposal, implementation and evaluation strategy, project outcomes) to manage key stakeholders and department.

  • Influencing, motivating, supporting, and enabling the team to work towards changes.

  • Responsible for auditing of pharmaceutical care practice in all clinics and develop quality improvement plan.

  • Support, facilitate, and participate in quality assurance activities within the country.

  • Participate in Quality Management Review process when needed.

  • Work closely with the Clinic Medical Director, clinical pharmacists, and Operations Director/Manager to assist, support and supervise the clinics in achieving overall quality goals and parameters.

  • Participate in country management meetings as required.

  • To be in the top tier for determining the educational and developmental needs of the clinical pharmacists and designing appropriate and useful content in cooperation with other departments.
