Activities:

Provide chronic dialysis therapy, including, but not limited to, the formulation of a dialysis plan with ultrafiltration, initiating dialysis through various access points, ensuring safety measures are met, and monitoring applicable vital signs for patients with chronic renal failure in accordance with the Care Plan.

Prepare and administer medications, fluid therapy, or other treatments as prescribed by the doctor, and perform the necessary documentation in the patient’s record.

Analyze patient data; assess and monitor the status of vascular accesses, review lab results, patient evaluations, and report any changes or abnormalities to the on-duty doctor.

Monitor the dialysis water treatment system for use according to established procedures.

Set up and check the functioning of hemodialysis machines, ensuring that all devices are clean, functional, and calibrated.

Evaluate the patient’s clinical condition before and after the session, including vital signs and vascular access.

Continuously observe vital signs and machine parameters during hemodialysis to ensure treatment efficacy and safety, adjusting blood flow and machine parameters as needed.

Conduct a detailed initial assessment, gathering information on symptoms, medical history, and recent changes in health status.

Inspect and care for the vascular access to prevent infections and complications, and provide the patient with home care instructions for the access point.

Maintain detailed records on treatment, including vital data, machine parameters, and any adverse events. Ensure documentation is complete and accurate.

Report changes in the patient’s condition and session results to the medical team and other healthcare professionals involved in care.

Ensure all procedures are carried out according to the standards and protocols set by Diaverum Brazil.

Provide ongoing education to the patient and their caregiver.

Proactively participate in quality improvement programs.
Requirements:
- Bachelor's degree in Nursing
- Registration with COREN
- Experience in hospital procedures and Hemodialysis
- Specialization in Nephrology (preferred)
